VOLUNTEER CHECKLIST FOR MOVING A STUDENT

When a student is moved, it is important to be sure communications are completed.

Chapter Responsibilities (Could be Liaison. Support person, President)

_________Notify staff Support Coordinator of name and address of new family.

_________Notify Area Team Chair, Support Coordinator of the move.

_________Notify High School about the family change.

_________Collect Host family handbook, student application from former host family
for the new host family.

_________Complete Host family application process for temporary or permanent family.

• Host Family Orientation
• List of AFS students and families for chapter
• Give new family name and address of previous family
• Give new family 3 Ring binder handbook
_________Check on bank account and assist student with a change if necessary.

_________Place new family on the mailing list.

Student Responsibilities

_________Give new address and phone number to his or her natural family.

_________Change of address with the Post Office.

_________Write a letter thanking the previous host family.

Area Council Responsibilities

_________Send a letter thanking the family for hosting.

_________Notify appropriate people of the move and new address.
